At Form Living Shop, we stand behind the quality of our contemporary furniture just as we value your satisfaction. If your purchase doesn’t meet your expectations, we offer a straightforward returns process for most items within 15 days of delivery.
Our Return Policy
We accept returns for most products including our dining tables, side & end tables, sofas, and stacking chairs when initiated within 15 days of delivery. All returned items must be in original, unused condition with all packaging materials intact.
Non-Returnable Items
Due to their specialized nature, outdoor tables cannot be returned unless damaged or defective. Custom orders and final sale items are also excluded from returns.
Return Process
- Initiate Your Return: Email our customer service team at [email protected] within 15 days of receiving your order. Use the subject line “Return Request – [Your Order Number]” and include:
- Your order number
- Item(s) you wish to return
- Reason for return
- Photos if the item is damaged
- Receive Authorization: We’ll respond within 2 business days with return instructions and a Return Merchandise Authorization (RMA) number.
- Package Your Item: Securely repackage the item in its original packaging with all included materials.
- Ship Your Return: Send the package to our Hollywood warehouse using a trackable shipping method. Return shipping costs are the customer’s responsibility unless the return is due to our error.
Exchange Process
For exchanges, follow the same process as returns. In your email, clearly state the item you wish to exchange for. We’ll process the exchange once we receive your original item.
Refund Information
Once we receive and inspect your return:
- Processing Time: 3-5 business days
- Refund Method: Original payment method (Visa, MasterCard, JCB, or PayPal)
- Refund Amount: Full item price minus original shipping costs (unless return is due to our error)
Note: International refunds may take additional time to appear in your account depending on your financial institution.

Damaged or Defective Items
If your item arrives damaged or defective, contact us immediately at [email protected] with photos of the damage and packaging. We’ll arrange for a replacement or refund and cover all associated shipping costs.
